National Health Insurance for foreigners in 2026: workplace and regional members, the six-month rule, the average premium and reductions
Last checked: 20 October 2026 · compared against 6 official sources opened that day
Korea's National Health Insurance covers foreigners too, but special rules decide when you join, how your premium is set and how family members are added. We checked Article 109 of the National Health Insurance Act and its Rules, the Ministry of Health and Welfare notice on health insurance for long-term foreign residents, and the National Health Insurance Service (NHIS) 2026 premium notice on 20 October 2026.
Three ways in
Article 109 sets out three routes: employees at a covered workplace become workplace members, others become regional members after living in Korea for a set period, and family members of a workplace member can apply to be dependants. Which route applies to you decides when coverage starts and how much you pay.
Workplace members
A registered foreigner working at a covered workplace becomes a workplace member unless an exclusion applies, and the employer files the enrolment. Check that health and long-term care premiums appear on your first payslip. Employee and employer each pay half of the earnings-based premium (Article 76); with the 2026 rate of 7.19%, the employee's share is 3.595% of monthly earnings.
Regional members and the six-month rule
Foreigners who are not workplace members become regional members when they have a qualifying status listed in Annex 9 of the Rules and have lived in Korea for the required period, which Article 61-2 of the Rules sets at six months. Some statuses join on the day they enter instead: permanent residents (F-5), E-9 workers, marriage migrants (F-6), and students covered by the notice, such as D-2 university students and D-4 students at primary or secondary schools.
How the six months are counted
The notice counts from your first entry. Time abroad totalling 30 days or less within the six months still counts as living in Korea, but a single trip abroad of more than a month restarts the count from the day you come back. Coverage starts the day after the six months are complete, so check the start date on your first bill.
2026 rates and the average premium
NHIS's 2026 notice gives a health insurance rate of 7.19%, a long-term care insurance rate of 0.9448%, 211.5 won per property point for regional members, and an average premium for foreign regional members of 158,630 won; the first two match the current Enforcement Decrees. Under Annex 2 of the notice, a foreign regional household's premium is calculated like a Korean household's, but if the result is below the average premium, the average premium is charged. The average is set from November figures and applies for the following January to December. Households headed by F-5 or F-6 holders are calculated on the normal basis only.
Reductions and households
Annex 2 reduces premiums by 50% for D-2 and D-4 students whose income and property are under the threshold (from March 2023), and by 30% for D-6 religious workers and certain G-1 humanitarian stay holders; combined reductions are generally capped at 50%. Each foreign regional member is treated as a separate household unless you apply to add your spouse and children under 19. Foreign family documents need an apostille or foreign ministry confirmation and a notarised Korean translation.
Paying on time
Foreign regional members pay in advance: Article 109(8) requires each month's premium by the 25th of the previous month, so November's premium is due on 25 October; first-month and late-month cases follow NHIS rules. If you fall behind, benefits can be suspended until you pay in full (Article 109(10)); under Article 9 of the notice, a premium due on the 25th that is still unpaid by the 25th of the following month means no benefits from the month after that. Ask NHIS about instalments before arrears build up. We could not confirm from official sources how arrears affect immigration decisions, so treat that as unverified.
Dependants and opting out
Family members of a workplace member can apply to be dependants if they meet the relationship, income and property tests and the residence period, but spouses and children under 19 are exempt from the residence period (Article 109(4)). People covered by equivalent medical protection under foreign law, a foreign insurer or their employment contract can apply to be excluded (Article 109(5)).
When coverage ends
Regional membership ends, for example, the day after your period of stay expires, when you become a dependant, or the day after you leave Korea if you stay abroad for a month or more while your permit is still valid. Report the loss of regional membership within 14 days (Article 61-2(4) of the Rules) and changes to your insurance details within 30 days. After an extension of stay, check that NHIS shows your coverage as continuing.
